Heartworm Walkthrough – Clock Tower

This walkthrough for Heartworm will take you step by step through Sam’s haunting descent into a long-abandoned house said to connect the realms of the living and the dead. From exploring its chilling corridors to unraveling cryptic puzzles and facing echoes of the past, this guide is here to help you uncover every hidden secret, survive every haunting, and immerse yourself fully in the game’s unsettling story. Let’s get started.

In this part of the walkthrough, we navigate through the Clock Tower and finally discover the true identity of the Tape Keeper.



Where is the Delta Emblem?

After the cutscene, grab the Delta Emblem in the entrance on the table by the fireplace.

Head upstairs and through the left side door into the dining room, and through the double doors on the right into the next hall. This room will introduce the new Headless enemies. They’ll often ambush Sam from above, and have the ability to leap over her to the other side. You know when they’re hiding above due to water dripping down.

If you head left and out onto the balcony area, you can’t quite get the item stuck in the cage yet, but you can enter the gallery by interacting with the single door on the lowest level opposite the side you came in.

Where is the first poem?

In the gallery is a poem – the keyword here is DWELLS.

Exit the gallery through the door on the other side, leading to the hall we were in previously.

Head left toward the next hallway. Go past the door with the missing piece until you get to two double-sided doors on either side.

The one on the left leads to the Stairwell. The first door on the bottom of the Stairwell leads to a Save Room. Save, then exit back to the hallway. The door on the right, when interacted with, will unlock and serve as a shortcut back to the main entrance way. Head back to the door with the missing piece that you passed earlier. Use the Delta Emblem on it to unlock it. It will bring you to the Hospital Lobby.

In the hospital lobby, head to the right wing and enter the door on the right to get to the Waiting Room.

Where is the solution to the hospital puzzle?

In the back, there are TVs with four words flashing, with a number associated with them. This is the clue to the puzzle on the third floor of the hospital.

Leave the Waiting Room and go through the door across from it to get to the Stairwell. Enter the door to the third floor Delivery Ward.

The puzzle here has four rooms that need to be matched with the words from earlier and then pressed in the order of the numbers. The correct order is 2 (couple watching TV), 3 (person falling through room with doors), 1 (Person sitting at desk), and 4 (person in a room with flowers).

Where is the Crucifix?

Enter the door at the end of the hall, go through the office, and enter the door that opened when you solved the puzzle. Grab the Crucifix from the crib. You will be teleported back to the Entrance Hall.

Place the Crucifix in the display case below the elevator. The third floor will now be accessible via the elevator.

Where is the second poem?

Head back to the Stairwell and go to the second floor. Head to the left and enter the door to find another gallery room. The word here to note is DESIRES.

Back in the stairwell, take the door to the right of the stairs.

Take a quick left and go through the door to return to the entrance hall, but on the 2nd floor. Take the elevator to the 3rd floor.

Where is the third poem?

Head through the left doors into the hall, take a right and enter the gallery room through the first door on your left. Here the word to note is DEPRIVES.

Where is the Multigrade Developer?

Head into the side door here and grab the Multigrade Developer from the far shelf.

Go back to the hallway and through the double doors at the end onto the balcony. Go all the way to the end of the platform and take the ladder down.

Where is the Beta Emblem?

Push the box into the empty cage. The opposite cage will rise and there will be a path you can cross on this level now to get down and grab the Beta Emblem.

Head through the double door you just passed, take the first door on your right, and use the Beta Emblem in the library to access the school.

Enter the school and navigate through the library, pool, and hall to the classroom at the end of the hall. This segment will introduce the Mannequin enemies to the game. They work very similarly to the Statues from earlier, but they don’t have a ranged attack and won’t trap you in an arena.

Where is the Videotape?

In the classroom there is a quiz you have to take on the TV. The quiz consists of seven questions, and there are no right or wrong answers. Upon completion, you’ll receive the Videotape and be teleported back to the entrance hall.

Place the Videotape in the display below the elevator. Back in the entrance hall, take the elevator to the 2nd floor and into the left hall. Take the door at the far end on the left and enter the dark room.

Use the Multigrade Developer on the undeveloped photo in the tray to receive the photo showing the path for the leap of faith.

Where is the Gamma Emblem?

Head to the first floor, the right hall, and through the door in the far corner to the “hole”. Use the photograph you developed to carefully walk across the gap and grab the Gamma Emblem.

Head to the first floor dining room and use the Gamma Emblem to enter the subway.

Where is the Lower Track Key?

In the subway, head downstairs to the first track level and onto the left side tracks. At the end of the track you can find the Lower Track Key.

Where is the Clock Tower Photo?

Run all the way to the other side of the tracks. Destroy the static barrier to get the Clock Tower Photo. Like the other photos, you won’t need it until you get back to the Archive, so feel free to drop it in an Item Box next time you see one.

Use the Lower Track Key on the padlocked gate in the middle of the platform. On the lower platform, head into the open train and to the door out the other end.

Go through the door to get to the Office.

In the Office, you have to deliver four colored papers to their matching colored receptacles. You can acquire the objects from the computer and printer in the middle of the office. You can use the colored signs above the gates to know if you are going in the right direction.

Enemies will begin hunting you as you do, stay alive. Additionally, when you go to deliver the yellow paper, you will get trapped in the room with the enemies for a minute, so be sure to avoid them. After you deliver the final paper, Work ID will appear in the copier, grab it and you will be teleported back to the entrance hall. Place the Work ID in the display case below the elevator.

Head back to the 3rd floor, to either hall and to the library balcony.

Where is the fourth poem and the Omega Emblem?

On the right side of the library balcony there is a gallery room. In the gallery, the note hint is DREAD. Interact with the emblem device and select the four words from the notes – DWELL, DESIRE, DEPRIVE, DREAD and take the Omega Emblem.

Head back to the right hall on the 3rd floor and into the garden room, and use the Omega Emblem to enter the gallery.

How to solve the sliding puzzle?

Go through the hole in that wall in the back of the gallery and enter the house. There are 11 Grandfather clocks, each stopped at a specific time, with an image etched on them. The time corresponds to the place that the image needs to be in the sliding puzzle upstairs. The puzzle is randomized so we can’t give an exact answer on how to solve it, however the solution should trigger when the puzzle looks like this:

Once solved, you can enter the room. Grab the Eulogy on the floating rock platform outside the wall Place the Eulogy in the display case below the elevator. Once all the mementos have been placed in the display case, head to the elevator. This is your last chance to explore the Clock Tower, so make sure you finish up anything you have left here.

The elevator should allow you to access the top floor now, head there and choose the “R” floor. Once at the top, go along the path until a cutscene starts. After the cutscene, take the Clock Tower Page from the cathedral steps. Go into the crypt next to the cathedral, which will bring you back to the Archive.

Archive

Use the Clock Tower Page on the tomb to start a cutscene. After the cutscene, you can grab the Ancient Key next to the door you came in from. If you got the Clock Tower Photo, make sure you use it at the machine to advance the good ending. Before leaving, this is your last chance to save the game before the final boss. Head back through the archive door. Use the Ancient Key to enter the door at the front of the cathedral.

Tape Keeper Boss

Unlike the other bosses, you can’t attack the Tape Keeper directly. Instead, you need to wait for them to become exposed.  At the start of the fight you need to dodge their attacks. This will include slamming on the ground, objects falling from above, and a sweeping attack. Keep moving!  

Eventually, the arena will turn purple, and four lanterns will appear in each corner of the arena. Take a picture of each one to stun the Tape Keeper. During this time, they will shoot projectiles at you, so don’t stay in one place too long.  

Once stunned, take a picture of the Tape Keeper to hurt it. Do this twice to finish the first phase of the boss.

The second phase, mechanically, is similar to the first: you need to wait for the lanterns, take pictures of them, then hit the boss when stunned. The boss has new attacks where film comes up from below to hit you, and film comes out of its chest to hit everything in front of it.

During the lantern phase, there is now a fifth lantern in the middle of the arena that you need to take a picture of. Once again, it takes two photos to defeat the boss and finish the game.
