Welcome to the first part of our walkthrough for Pathologic 3.
You play as Danil Dankovsky, a Bachelor of Medicine obsessed with death and the concept of immortality. The game starts with you traveling to a remote, backwater town, where a supposedly immortal man, Simon Kain, resides. The first part of our walkthrough begins as soon as you arrive at the train station.

Finding the Experimental Prototype

At the end of the opening cutscene, you’ll see a few children run off with your luggage from the Train Station. Press J once the game begins to open your Mindmap.
You’ll see a radial menu which depicts all 12 days. Hovering over each day gives you a short note on what happened on that specific day. For now, you’re stuck at Day 1. Click Day 1 and hover over the nodes.

Immortal Man is your current objective. Clicking on the Are You Serious node opens the Map. Close the window.

Talk to the man sweeping the train platform.

Run through the dialogues. You’ll have two to three different dialogue options to choose from during the conversation.

After talking to the Street Sweeper, you’ll be introduced to the Reasoning system. You basically have to connect different pieces of information logically to turn them into Thoughts. Here are the ones that we went with:
- This town gave me a warm welcome, didn’t it?
- The street sweeper says no one here will help me find the missing items.
- Follow the thieves’ trail of ripped suitcases
- The most important thing is the microscope and the experimental prototype.
This unlocks a new Thoughts Utopia and No Help at All in your Mind Map. Your goal is clear: track down the kids and retrieve your missing equipment.

Head left past the Street Sweeper to spot a child peeking from around the corner. Speak with him.
The conversation with the kid gives you some more information about the children who ran off with your stuff. You’ll know that you can find them at the “Face of the Train.”

By now, you’ll have unlocked the Concentration system. On the keyboard, press and hold Q to activate Concentration, which shows you a trail that leads to your goal.


Along the way, you’ll find some of your suitcases that you can loot for different materials. They will let you manipulate your Apathy/Mania meter.

Continue following the trail to the front of the train to find a group of children. Talk to them.

If your Mania meter is high, the kids will be scared, and you won’t be able to talk to them. In that case, use some items that increase Apathy in your inventory to “cool down first.”

Run through the dialogue with the children, and you’ll eventually find yourself in a flashback in The Capital back at your lab. This segment serves as a tutorial for the Experimental Prototype.

Talk to the doctor, Platon, standing in front of you. He’ll tell you about a patient who has had an apparent resurrection in the morgue. But to reach them, you need to get past the miasma. This is where the Experimental Prototype comes in.

This leads to another Reasoning segment. Here’s what we went with:
- Pneumatics with mechanical priming.
- Clears the miasma in front of it.
- Need to spend fluid wisely-the morgue corridors are like a maze.
- To load the prototype, I need a stimulant, an anesthetic, and a toxin.
- They should be here, in those beige cabinets…
- Add the ingredients one by one into the setup, and heat it up…


After this segment, you’ll have the experimental prototype in your hand. Loot the Beige Cabinet up ahead and interact with the table to load all three materials into the prototype.

The Experimental Prototype, once loaded, lets you disperse the miasma (black fumes) from the area. But you’ll only have 20 charges available when it’s loaded. On top of that, the miasma doesn’t remain dispersed forever. So you’ll have to be quick.

Make your way through the morgue till you spot a woman sitting on a chair. This is the resurrected patient Platon was talking about. Speak with her. Choose a dialogue to complete the flashback segment.

You’ll find yourself back in the village talking to the children. Run through the dialogue, and you’ll learn that they have your Experimental Prototype, but your Microscope is with another group of children called the Dogheads.
To find them, you need to make your way to a place called the Stairway to Heaven.
Reaching the Maw

From the front of the train, go forward and turn left. You’ll see a wierd person standing there. You can talk to him.

This is Odongh. Run through his conversation, and you’ll unlock a new Thought Lumpy Man in your Mindmap

After the conversation, head back and go left to find a group of people surrounding a dead body. Talk with the woman kneeling down.

The Daughter of Suok (woman with torn clothes) will ask for the body. Run through the dialogue, and eventually you’ll have three options.
- Do not give the body (option 1)
- Give the body (option 2)
- You don’t care (option 3)
We went with option 1 and unlocked the Golem of Bones Thought in the Mindmap.

Continue moving, following the Concentration trail along the train track. You’ll find a couple standing near a bunch of coffins. Talk to them. This will unlock another Thought in the Mindmap.
After the conversation, head left, through the open gate to reach the Maw.
Finding the Stairway to Heaven

Continue moving forward till you reach a town square

As you enter through the gate on the left, you’ll see a festival. A group of Daughters of Suok will be dancing around a bull. You can speak with them for some additional dialogue and a new Thought.

Odongh will ask you to go further to see their celebrations and tell Guuga afterwards.


Continue along the path, and the scenery will change suddenly. Keep moving forward.

Near the gate at the end of the path, you’ll find Odongh again. Talk to him, and you can tell him what you saw. We decided to just hurry off to find the Microscope. Sharing what you saw gives you an achievement.

Keep walking along the paved path, and you’ll spot a trio of strange-looking people standing up ahead. Talk to them.

If you decide to speak with them and run some lines with them, you’ll get a new Thought The World’s a Stage.

Continue past the actors, and you’ll find yourself in front of a floating staircase. This is the Stairway to Heaven.
Retrieving the Microscope

As you get close to the staircase, you’ll get the Impossible Geometry Thought in your Mindmap. Speak to the child sitting near the base. He’ll tell you that Phil, the child who took your Microscope, is standing on the stairway.

Head up the stairs till you spot a group of kids wearing Dog Masks gathered around your Microscope. Speak with them.

Run through the conversation, and eventually, they will agree to a trade. You can trade the Pocketwatch from your inventory for the Microscope.

This leads to another flashback sequence, and you’ll find yourself back at the Thanatica Institute in The Capital two weeks ago. Talk to the Spontaneous Resurrectee and Doctor Serafima.
Run through the conversation, and you’ll have to perform three tasks:
- Examine the patient (option 1)
- Use the Microscope (option 2)
- Examine the patient’s belongings (option 3)
The order doesn’t matter. Choose any of the three options to get started.
Examine the Patient (Option 1)

Choosing option 1 will give you a close-up view of the patient’s body. You need to examine the different bruised parts of her body.
Rotate the camera and press Q to enter Inspection Mode. Then, hover over the bruised parts to examine the patient.

Do this for all the parts of the body.
Using the Microscope (Option 2)

Choosing the second option will open a new window where you have to place Tissue Samples on glass slides. Press Spacebar to take a sample.

This leads to another window where you have to add Reagents to the samples and look under the Microscope. Place the sample into the empty slot and press Spacebar to examine.

This takes you to the Microcosm window. You now have to adjust the magnification using W/S and the Mouse Scroll Wheel until you find the Red Bacteria. There is a timer at the top, and if it reaches zero, you need to take another sample and use another Reagent.
When you find the Red Bacteria, let the bar at the bottom fill up to complete the segment.
Examine the Patient’s Belongings (Option 3)

Walk to the back of the room, and you’ll find the patient’s belongings on a tray on the last table. Press E to concentrate on the objects.

This segment is similar to the Reasoning sequences from earlier. You need to examine the objects and choose logical options to connect the pieces. Here are the options that we went with:
- Our Lazarus’ personal effects, from the morgue.
- Two rings; obviously a pair. One would be too big for her.
- Jilted Fiance?
- Herbs. Not a bouquet; looks medicinal
- Self-medication with folk remedies?
- Papers. They look official.
- The description of the treatment is vague-something about novel drugs.
- Morphine; tourniquet
- Or suffered from severe pain?

After this, there will be two more dialogue options, and you’ll find four options to choose from to complete the segment of your diagnosis. We went with:
- Apparent death – reaction to experimental drug? Lab got scared, didn’t sort it out?

Return to the doctor and the patient and speak with them. Doctor Serafima will ask for your diagnosis. You have four options. We went with the second option and diagnosed that she was on a drug trial at Thanatica.

After the conversation, you’ll return to the kids wearing Dog Masks. Run through the dialogue, and you’ll suddenly see a figure of a man in the sky. He’ll start speaking. After a while, you get transported to a different timeline.
The Inspector

You’ll find yourself in a dark room with the man that you saw in the sky standing in front of you. Talk with him.

This is the Inspector of the Special Committee sent to investigate your crimes. As Danil notes, he is your enemy.

During the conversation, you’ll notice that he keeps dismissing your story and keeps guiding you toward the end, where you supposedly abandoned the town in the middle of a plague. Continue running through the dialogue, and eventually the segment will end.
This completes Day 1 and sends you directly to Day 5 in the village.
