Welcome to Into Indie Games’ walkthrough for the second part of The Sinking City 2.
After performing the ritual, Faye Bennet’s body gets snatched away by a slither, leaving her spirit behind. As Calvin and Faye set off to bring her body back, they arrive at the Akeley Memorial Hospital.

Locating Faye’s Body

After the cutscene, you’ll arrive at a forest path. Continue heading up the path.

You’ll arrive at the entrance of a big building. As Calvin points out, this is the Akeyley Memorial Hospital.

There’s a container near the entrance to the right that you can loot. Faye will point out a hole in the roof of the building, guessing that’s where the Slither entered.

Head in through the gate and walk up to the door. The main door is unlocked, so you can step right in.

You’ll hear an announcement as you enter the Hospital’s Reception. Take a moment to get your bearings. There’s a door with a device showing a woman’s face. These are Mirror Doors, and you’ll find a variety of these throughout the zone. We can’t access the door right now. There’s also an E.R.C on a bench near the Mirror Door.

Head in through the door to the Safe Room and save your progress.

Return to the Reception and take the door that leads to the Founder’s Hall. Before you do so, make sure you collect the Eye Technology Codex Entry from the notice board.

Through the door, you’ll spot a locked door on the left that leads into the Pharmacy. This requires the Caduceus Key to unlock. Head up the stairs through the door at the far end.

Go all the way to the top to trigger a cutscene as you find Faye’s body, suspended inside a red energy sphere.
Finding the Prism in The Anatomical Theater

After the cutscene, take a look around at the room. The Staff Room has another Mirror Door, so that’s not accessible. Look for a small passage with an open window you can climb out of nearby.


Head through the door at the balcony to reach the Upper Hall. Take out the enemies as soon as you enter. There’s another Mirror Door here.

Go through the doorway in the center and take the stairs to the lower floor.

Pick up the 1st Floor Plan Map from the board and unlock the door to the reception to open a shortcut to the Safe Room.


There’s a double door here that leads to the Anatomical Theater area where Faye Bennet’s body is hovering. Look around for a small door that leads into the Locker Room. You’ll find a Codex Entry (Letter for Intern Dexter) and a Regenerine Tonic here.

Head through the double door leading to the Anatomical Theater area. As you go forward a few steps, the door to the Anatomical Theater closes.

Turn left and enter the Treatment Room. You’ll spot a weird object shrouded in blue light near the wall. Pick up the Station Manual + Occult Symbol evidence from the table on the left. Then interact with the object.

You can’t do anything with it right now. But it will get marked as a Station on your map.

Pick up the Prism System Schematic evidence from the board on the wall on the right near the Station. The evidence tells you of three Stations located in the Treatment Room, which is the one you found, the Chapel, and the Basement. The markers for these Stations will get added to your Map.

Interact with the half-closed shutter in the Treatment Room to move over to the Anatomical Theater.

An announcement will tell you that you need to reset the protocol using a Charged Prism. Interact with the object near the energy sphere.

The Prism has a shield around it, so you can’t grab it right now.

Head to the other side of the room and enter the door beside the headless body.

Pick up the Prism Station Reset Protocol evidence from the table. Then interact with the Switch inside the room. Combine the Prism Station Reset Protocol and the Prism System Schematic in your Investigation menu.

Doing so powers down the energy shield around the Prism. Pick it up.

As soon as you do so, all hell breaks loose as Slither Pawns rush in at you from everywhere. Faye tells you to get out, and that’s the best thing you can do. There are too many for you to fight.

Head toward the Treatment Room. The barricaded door to the Emergency is now open. Head along the passage.

When you reach outside, turn around and go across the wood scaffolding.

A cutscene triggers. An enemy grabs you through the window, and both of you fall down to the Lower Yard below.
Finding Nurse Kitty’s Face (Woman’s Face Mask)

After the cutscene, head through the door up ahead.

There’s another Mirror Door leading right. Pick up the On Mirror Doors Codex Entry from the table near it.

Continue along the passage to the left. As you progress, you’ll see a body get flung to a wall in front of you.

You’ll also catch a glimpse of something sinister lurking in the shadows. But you can’t do anything about it right now. Head through the gate up ahead and continue forward. You’ll spot a door with a rusted doorknob and a room with another Station. There’s a gap you can squeeze through at the end.

You’ll encounter two more enemies as you step forward. Take them out. Continue past the enemies until you reach the door to the Morgue. Head inside.

Pick up the Morgue Worker’s Memo Codex Entry from the table. There’s an E.R.C on the table just up ahead.

You can interact with the Mortuary Chambers to pull them out. Pull out all the Chambers and collect the items inside them.

Inside the final Mortuary Chamber, you’ll find a locked storage box holding one of the faces you’ve been seeing on those Mirror Doors. This one is Nurse Kitty‘s face.

Rotate the storage box and interact with all the points to unlock it.

This will trigger a cutscene as Dr. Thadeus Lowe, the Head Surgeon, tells you not to touch the Face. She seems to know a lot about the energy sphere, aka the Eye, and what’s happening with Faye. But he doesn’t want to help you.

Once the Head Surgeon leaves, the bodies in the Mortuary Chambers attack you. Take them out.

Pick up the Woman’s Face Mask and head through the door.

You’ll spot a Mirror Door on the right. Pick up the Basement Plan from the board. There’s a shortcut door here that leads back to the Lower Yard. Unlock the door, then head into the Safe Room nearby first to save your progress.
Charging the Prism at the Blue Station (Treatment Room)
Finding the Second Occult Symbol

Leave the Safe Room and head to the Lower Yard through the shortcut door you just unlocked. Then head through the door on the other side to loop back to the Mirror Door with Nurse Kitty’s Face.

Interact with the door to trigger a cutscene as Calvin puts the mask on the Night Shade and uses it to disable the door’s security.

Go through the door and up the stairs to the Prosthetics Lobby. You’ll see a strange glow coming from a poster on the wall. Interact with it.

This glow will materialize into a blue Occult symbol. This will be added as Evidence to your Investigation menu.
Obtaining the Anti-Rust Acid

One of the doors in the Prosthetics Lobby leads to a bathroom where you’ll find an E.R.C Activator Pen. Use it to unlock the E.R.C in the Morgue and unlock two more slots in your Inventory.

There’s another door here that leads outside to the Therapeutic Garden. But don’t take the door just yet.

Instead, go up the stairs in the Prosthetics Lobby and enter through the door at the top to reach the Workshop.

As you enter, you’ll spot another Mirror Door on the left leading to Materials. You can’t use it right now.

There are plenty of materials to loot in the Workshop. The Anti-Rust Acid can be found on the table near the board with the X-rays. Pick it up.

Also note the Prosthetic Workbench near the windows. This is used to repair Face Masks, which you’ll need later.
Obtaining the Third Occult Symbol

With the Anti-Rust Acid in hand, go back downstairs and head through the door that leads to Therapeutic Garden. There are a few containers you can loot behind you.

Continue forward along the garden. There are some Stygians here that you’ll want to take down.

Once it’s safe, head to the far end of the garden. You’ll spot a door leading back into the hospital, but don’t go in. Instead, head to a small corner past the door shown in the image above.

You’ll see another glow coming from a bear trap. Interact with it to get the third symbol.
Reaching the Treatment Room Station

Head to the door that leads to the Central Staircase from the Therapeutic Garden. You’ll have to use the Anti-Rust Acid to open the door.

The door on the left leads to the Treatment Room.


Run back to the Station and interact with it. Insert the Prism, and the slots around it will light up with Occult symbols. Note that the top triangle in the prism is the only one that’s lit.

Open the Investigation menu and combine the three evidence with the matching blue Occult symbols. The symbols seen on the evidence are the ones you need to use for this puzzle.

You need to align the three symbols in the row that matches the blinking blue light on the Prism. Check the image above for the correct solution.

Once you validate the puzzle, the Prism gets charged. Grab the Partially Charged Prism from the Station.

As you exit the Treatment Room and walk back toward the Reception, you’ll see Dr. Lowe, walk by up ahead. He won’t stop for you, but a bunch of enemies will spawn nearby.

You could fight them off or run back to the Reception. The choice is yours.

However, as soon as you enter the Reception, an event triggers as you get your first look at a Shadow Entity. You can’t fight them, and they will spawn randomly in different areas.

All you can do for now is run away. Run into the Safe Room at the Reception. It can’t enter the Safe Room. Save your progress.
Obtaining the Old Man’s Face Mask
Finding the Right Half of a Face

With the Shadow Entity roaming the halls, you’ll have to be careful as you progress through the next parts of the area. Leave the Safe House and go through the Mirror Door in the Reception. This takes you to the east wing of the Hospital.

There’s a set of stairs heading up on the left leading to the upper floor. For now, head right along the passage to the waiting area.

Head into the open room at the far end of the waiting area. You’ll find the Memo: Repairing the Key Codex Entry here.

Go into the adjacent room, and you’ll find one half of the Old Man’s Face Mask in a storage box.

Pick it up and interact with all the points to unlock the box. Collect the Right Half of a Face.

Make your way to the start of the east wing. Take out the monsters that now roam the hall.
Finding the Caduceus Key

Go up the stairs that you saw earlier. Pick up the 2nd Floor Plan Map as you reach the top.

The Head Surgeon‘s room is locked with a Mirror Door. Enter the Outpatient’s Room through the door.

You’ll find the Note with Digit ‘3’ evidence on a table. Go back outside.

Continue along the hall. The Isolation Ward’s door requires the Caduceus Key. You can find a Hasty Note on a table nearby. The note mentions the Caduceus Key being in the west wing inside the Staff Room of the hospital.

Continue past the door and head out to the balcony. You can enter the Observation Ward through an open window. You can also unlock a shortcut back to the Upper Hall from the balcony.

You’ll see a familiar glow coming from a poster on the wall. Interact with it to get a pink Occult symbol. This will come in handy later.

Unlock the Observation Ward door from the inside and make your way back to the Reception and head up to the Founder’s Hall. The Mirror Door in the Staff Room can be unlocked now that you’ve got the Woman’s Face Mask.

You will find the Folded Note and the Stuff Memo: Incursions Codex Entries inside the Staff Room. Head outside to the balcony.

Pick up the Caduceus Key from the table.
Finding the Left Half of a Face

With the Caduceus Key recovered, the Isolation Ward can now be accessed. But first, head down the stairs of the Founder’s Hall and open the door to the Pharmacy. You’ll find a Lifesaver Kit, a Radio Codex Entry, and another pink Occult Symbol here.

Go to the east wing and enter the Isolation Ward next. There are doors on both sides of the passage.

Check each room. One of them contains some writing on the wall. Inspecting it gives you a Codex Entry.

There are some crafting materials and healing items in another room. The last accessible room gives you a yellow Occult symbol.

At the end of the passage, you’ll spot a hole in the floor on your left. There’s a door on the right. Drop down through the hole to reach the General Ward.

First, deal with the monsters as you drop in. Then loot the area. You’ll find an E.R.C here along with a Codex Entry (Torn Page From A Diary).

Head out through the door. You’ve arrived at the other side of the Therapeutic Garden.

Turn right and pick up the Letter with Digit ‘1’ from the table.

Continue forward and go down the steps. There’s a closed gate up ahead, and a path through the gate on the right that leads to a sitting area.

You’ll find a Radio, an Inventory Upgrade, and some crafting materials here. There’s also a shortcut gate you can unlock that connects to Therapeutic Garden.

Head through the gate straight ahead. Turn left to spot a ladder. This ladder will take you back to the room with the hole in the Isolation Ward.

Continue forward along the path. You’ll see something big walking away.

Continue forward and pick up the Codex Entry (Lost Letters – Goodbye Maria) on the left. Then head up to the yard up ahead to trigger a cutscene. You’ll have to take it down before you can move on.

The biggest thing to know about this fight is not to get caught in the open. The monster creates poison puddles that can deal a lot of damage if you get caught in them. To kill the monster, keep firing at its weak point on its belly. The Minutemen are excellent for this fight.

When the fight is over, head into the Prosthetics building through the door. You’ll find the Left Half of a Face on the table up ahead. Unlock the storage box to collect it.
Repairing the Masks

Unlock the door on the left to open the shortcut back to the Prosthetics Lobby. Then go up the stairs into the Workshop.

Interact with the Prosthetics Workbench and place the two pieces of the mask. Pick up the Old Man’s Face Mask afterward.

Enter the Materials through the Mirror Door. Then go out through the next door. Pick up the Black Reef Longshot from the table up ahead along with the Note with Digit ‘0’.

Turn left and head through the door. Take out the enemy on the stairs and head to the Therapeutic Garden from the Prosthetics Lobby.
Investigating The Wireless Room (Optional)

As you arrive at the Therapeutic Garden, you’ll hear another announcement, this time talking about the Wireless Room. Faye suggests checking out the room.

To reach the Wireless Room, you have to take the Mirror Door in the Upper Hall area. Check the map and plot the best route for yourself. Be careful of the enemies roaming around.

After reaching Medical Records, pick up the For Akeley’s Widows, Questions Linger Codex Entry from the stack of boxes.

Turn right and squeeze through the filing cabinets. Take out the monster here. Then interact with the glowing area to reveal another yellow Occult symbol.


Head through the door. You can unlock the door on your right to connect the zone to the Upper Hall. Head left and turn the corner to arrive at the Wireless Room.

As you enter, you’ll see three brains in metal boxes. There’s a wooden cabinet with buttons and dials in front of it. Let the dialogue play out.

You can then interact with each of the three brains. They will tell you what the primary and secondary frequency should be, along with their comment about the other brains.

Talk to all three brains. Then open the Investigation menu and connect all their testimonies and the Note About Wireless Room to earn a Dream Essence.

Interact with the wooden cabinet next. You need to compare the testimonies of the three brains to find the solution to this puzzle.
The correct solution is 13 | 05 | 36.

Solving the puzzle rewards you with a Dream Essence.
Loot the area before moving on for an E.R.C Activator Pen. I used it to unlock the Cutter’s Tomahawk Shotgun Upgrade from the E.R.C in the Reception.
Charging the Prism at the Yellow Station (Chapel)

So far we’ve gathered two yellow Occult symbols. With the Old Man’s Face Mask in our hand, we can make our way to the Chapel, where we can charge the Prism at the yellow station. But we still have to find one last Occult symbol.
To reach the Chapel, head to the Mirror Door at the Waiting Area on the east wing. It’s a straight sprint from the Reception.

Go through the door to arrive at the Chapel Courtyard. Enter the Chapel through the main door.

You will find the Lost Letters – Deja Vu on a bench on the right as soon as you enter.

There’s a door on the right that leads straight to the Station. But before you go there, check the dais at the front of the Chapel. You’ll find the last yellow Occult symbol here.

Head into the Priest Room on the side and pick up the Unsent Letters from the table. Connect all three yellow Occult symbols in your Investigation menu for a free Dream Essence.

Interact with the Station and line up the correct Occult symbols with the yellow triangle. Pick up the Partially Charged Prism afterward.

Head out to the Chapel, and you’ll see Dr. Lowe walk by to the back room. But as Calvin tells him to stop, something strange happens.

Follow Dr. Lowe into the Sacristy. You’ll see rows of brains stacked on shelves on all sides. Continue forward and pick up the Local Residents Codex Entry on your left.

Squeeze through a gap, and you’ll find the Head Surgeon’s Face Mask on a table up ahead. Pick it up to trigger a cutscene.

After the cutscene, leave the Chapel through the main door. You’ll have to fight a wave of enemies in the main part of the Chapel.
Run all the way back to the Reception and save your progress. There’s only one Station left, and that is in the Basement.
Charging the Prism at the Pink Station (Basement)
Finding the Vertebral Key

Head to the central staircase (shown in the image above) and take the stairs down.

You’ll reach a locked door. This requires the Vertebral Key. The note on the wall says the Head Surgeon restricts access.

But now, since we have the Head Surgeon’s Face Mask, we can enter the Surgeon’s Office on the second floor of the east wing. Take the stairs up from the Central Staircase and loop around to the Mirror Door.

As you enter, you’ll hear some voices in your head. Pick up the Diary of Thaddeus Lowe from the table after heading inside.

The Vertebral Key is on the desk. Pick it up as well.
Obtaining the Third Pink Occult Symbol

There’s a wardrobe on the wall that you can push out of the way. Doing so reveals a gap that lets you enter the Nurse’s Station.

Enter the Nurse’s Station to pick up the final E.R.C Activator Pen in the zone. You can use this to open the chest in the General Ward for a Dream Essence.

There’s also a Note with Digit ‘8’ evidence in the room. This is the final Note with Digit evidence. You can connect all 4 for a Dream Essence. The code comes out as 1308. You’ll need it later.

Head down the stairs in the Central Staircase and use the Vertebral Key to open the door. You’ll catch a glimpse of the entity you encountered in the Chapel up ahead. However, it will disappear once you get close.

As it fades away, you’ll spot a familiar glow up ahead. Interact with it to get the final pink Occult symbol.
Reaching the Basement Station

Head into the Maintenance Room through the gap in the door.

You’ll see a nightmare monster as soon as you enter. But it seems docile for now. Make your way around the room.

As you reach for the exit, the monster comes alive. Take it down. Like before, attack the weak point on its belly to deal more damage.

Head into the Manipulators once the monster is down. You’ll find the final Station here. There’s also a Radio Codex Entry you can unlock in the room.

Insert the Prism and choose the correct symbols. Then pick up the Fully Charged Prism.
Heading into the Storage (Optional)

In the basement area, there’s a door that can be opened with the Anti-Rust Acid. Head over to it now and open the door.

This will lead you to the Storage. The gate on the left is locked with a combination code. You already have the code from the Note with Digit evidence. Enter the code 1307 to unlock the gate and head in.

The area contains plenty of ammo and resources, but the most important thing here is Doyle’s Cold Blood upgrade for the Minutemen.
The Ending


With the Prism fully charged, we now have to find our way back to the Anatomical Theater. The path is through the Mirror Door in the Basement.

Head through the door and take the stairs up to reach Faye’s suspended body. Place the Prism into the Eye to trigger a cutscene.

Once the Slither runs off with Faye’s body, follow it. Continue moving forward through the zone, running toward the boat. Ignore the monsters for now.

You’ll eventually jump off a ledge into an area full of Slitherworms. As you head toward the exit, the Slitherworms block your path. But Faye thinks she can open it if you buy her enough time.

At this point, you’ll have to fight off waves of enemies as Faye concentrates on breaking the barrier. There are refilling containers in the area that you can loot for ammo and healing items.

At the last wave, you’ll have to fight one more nightmare monster. Once you take it down, the barrier opens.

Continue moving toward your boat. A cutscene will trigger along the way.

When the cutscene ends, jump off the ledge and get into your boat to complete this zone. Our next stop is the Arkham Fish Market.
